Patient PortalWelcome to our Patient Portal Please click here to log-in to your patient portal where you can update your contact information, health history and other required patient forms, such as consents for procedures. You can log-in to the Patient portal using a one-time code or make an account. If you already have an account using the app, you will be given the choice to log in with your account information. FORMS: Once you are logged into the Patient Portal you should land on the Patient Forms tab, you will see your name and any other family members on your account listed. Under each member's name you will see their next scheduled appointment. Next to each member, there will either be a drop-down menu or a number with a "required" notification that will appear when the member has forms to update. (If you do not have any required forms to fill out but recently moved or switched employers and have new insurance, select the drop-down menu and you can update your contact and insurance information). PAYMENTS: To make a payment, login to the Patient Portal. After logging in, click the Payments tab at the top of the webpage and you will see your account balance (this includes balance for all members). Enter in your card information, payment amount and submit your payment. Please note, you will be given the option to name & save your card on file to use for future payments. |
